What do we know about one of the latest nicotine products on the market? Not enough to know if they are helpful or harmful. Nicotine pouches contain a mixture of synthetic or tobacco derived nicotine along with sweeteners, stabilizers, and flavors. These pouches are tobacco free and do not require inhalation. Tobacco free does not mean risk free. Some brands deliver nicotine at levels as high as smoked tobacco. Nicotine concentrations range from 3mg to 9mg per pouch. The nicotine is absorbed through the lining of the mouth and throat.
Uptake in the use of nicotine pouches is rising. Less than 1% of adolescents were using nicotine pouches in the year between 2022 and 2023 but between 2023 and 2024 pouch use by 10th and 12th grades rose from 2.4% to 4.6%. People who smoke and vape are using these products to help stop their smoking habit but there is little scientific data that these products promote smoking cessation. The primary health concern involves the potential for these products to attract nicotine-naïve populations, particularly adolescents. Remember these products are associated with nicotine addiction and can lead to other more harmful behavior.
On the risk continuum, nicotine pouches are probably less harmful than cigarettes and vaping for people who already smoke but the use carries risk for GI issues, oral health problems and cardiovascular toxicity.
Right now, I cannot recommend these products to smokers or vapers, and I certainly feel no one who has never used nicotine should try products.
